What you need for WM 16: Treatment Works Plan Approval for New/Modified Facility associated with Surface Water (NPDES) Individual Permits
This plan approval serves to protect the public health, welfare and the environment through the control of pollutant discharges to surface water. It is required for any major physical modification to a treatment works which does not require a modification to the permit issued to the facility.
Use this permit if you have a treatment works with a surface water discharge (314 CMR 3.00).
You may use this category to modify the treatment works, provided no modifications to the existing permit are required.

Claim your Authorization
- If you have an existing NDPES permit, you will first need to do one important step: Claim your Authorization. This process will link your Plan Approval (WM 16) to your existing permit and will verify your identity.
You may fill out and save the Microsoft Word or Adobe Acrobat PDF "NPDES Claim Authorization Form" on your computer. This will also be uploaded as an attachment during the Claim process. - Claim Online via the EEA ePlace Portal.
Note: if you are a first-time user, you will need to begin by creating an account.
