Virtual Gateway Information

To access BSAS eLicensing on the Virtual Gateway for SUD programs, your virtual gateway access administrator will need to complete the BSAS eLicensing Provider Request Form.

• Instructions:
  1. All non-role fields are required
  2. Fill in the form, place an “X” in the “BSAS Program Role” to access your program(s) BSAS licensing application(s),
     • Place an “x” in the “BSAS appeals role” ONLY if your program is a licensed OTP and the Program Director has approved your access
  3. Save document as YourOrganizationName_MMDDYY, do NOT save the document as a PDF. It must be sent to the Virtual Gateway as an excel document.
  4. Email the completed form to: [e-mail]
  5. Please submit one form per email
