LTSS Assessment Overview

The LTSS Assessment is a uniform assessment used for all MassHealth members enrolled in or seeking eligibility for the Community Case Management (CCM) program.  The LTSS Assessment determines a member’s need for CSN services by reviewing and quantifying the time it takes to complete all medically necessary nursing interventions.

Other MassHealth and community-based services and programs

The CCM Clinical Managers can help with authorization and coordination of other MassHealth services. These services may include personal care attendant (PCA), home health aide, durable medical equipment (DME), oxygen and respiratory equipment, medical supplies, and therapy services. The Clinical Manager works with physical, occupational, respiratory, and speech therapists, pharmacists, and social workers. This team of specialists help coordinate other MassHealth services such as DME, oxygen and respiratory equipment, orthotics and prosthetics, and therapy services.
