Role of your CCM Clinical Manager
Your Community Case Management (CCM) Clinical Manager can help you determine what may be the best approach to filling your CSN hours. In some cases, a schedule change, a change in CSN providers, or working with many CSN providers (co-vending) may help. Some Members may need to use other MassHealth services, such as skilled nursing visits, until their nursing hours are filled.
The CCM Clinical Manager can assist you with enrollment into the CSN Nurse Directory and will also provide you with a list of CSN agencies and IN providers, including contact information, and can assist you in contacting CSN providers and determining who might be available to provide CSN services to you.
The Online CCM Nurse Directory
The CCM Nurse Directory is an online portal to help match CCM members and families with available CSN nurses. CCM Members and or their representatives will create profiles, identifying their scheduling requirements, care needs, and other nursing preferences. Independent Nurses and Agency Nurses will have profiles as well, detailing their skill sets, geographic location, and availability. CCM Members and families will be able to search for nurses based on their care needs and time requirements. CCM Members and families will be able to communicate with CSN providers directly through the online portal.

CSN Provider Types
