The following codes are included in the MassHealth ACO Primary Care Sub-Capitation Program. As such, a practice eligible for the Primary Care Sub-Capitation Program will not be paid fee-for-service for these codes for members attributed to their tax identification number (TIN) and when delivered by a qualifying provider.
