This form is used by both parties in any divorce, divorce modification or separate support case to voluntarily request to participate in the Early Case
Settlement Process. This form must be filed within 60 days of the filing of the initial complaint. For more details, see section II of the Standing Order.

This form must be used to request a settlement conference when the parties are participating in the Early Case Settlement Process. For more details, see section II (C)(4) of the Standing Order.
