The purpose of this Order is to provide for the collection of non-clinical identifying information in commitment proceedings in the Juvenile Court Department necessary to comply with the courts’ reporting requirements pursuant to Chapter 284, Acts of 2014, An Act Relative to the Reduction of Gun Violence.(1)

IT IS THEREFORE ORDERED: 

An applicant seeking a court order to commit a respondent to a mental health facility pursuant to G.L. c. 123, § 12(e), a petitioner seeking a court order to commit or retain a respondent for inpatient care pursuant to G.L. c. 123, §§ 7 and 8, and a petitioner seeking a court order to commit a respondent for treatment for alcoholism and/or substance abuse pursuant to G.L. c. 123, § 35, shall provide the respondent’s date of birth and social security number, if available, to the court. 

Contact

(1) Although commitment matters are also brought under G.L. c. 123, §§ 15(b)16(b) or (c), or 18 that arise out of criminal proceedings, the respondent's or defendant's date of birth and social security number is obtained and provided to the court through the Defendant Identification Information System in those cases.
