UIC technical compliance forms are now available for all UIC Class V well types for which the submittal of an eDEP BRP WS 06 UIC registration application is required. The main purposes of these forms are to assist applicants to determine whether a proposed UIC Class V discharge is approvable under current MassDEP UIC regulations (310 CMR 27.00) and to allow for quicker issuance of registration approvals. These technical compliance forms are not currently available through eDEP; however, eDEP applicants should complete the applicable UIC Technical Compliance form and either attach an electronic copy to the eDEP UIC registration application or mail it to MassDEP. Technical compliance forms are not required if you are registering an existing UIC well for the purpose of closing the well.
Technical Compliance Form for All UIC Well Types
The form below should be completed and attached to eDEP BRP WS 06 UIC Registration applications for all UIC well types for which none of the other four well-type-specific UIC technical compliance forms apply (see other technical compliance forms below).
Technical Compliance Form for Public Water Supply Facilities
The form below should be completed and attached to eDEP BRP WS 06 UIC Registration applications that are submitted by Public Water Supply Facilities for the following UIC well category/well type combinations: “other/special drainage - groundwater infiltration” and “water purification/public water system”.
