Unemployment fraud reporting form

If you believe someone has used your identity information to file a fraudulent UI claim or you believe your UI account may have been compromised, you can report it to DUA by completing the fraud form below.

As part of a national unemployment fraud scheme, criminal enterprises are using stolen personal information to try to file fraudulent unemployment claims. If you believe someone has applied for unemployment benefits using your personal information, use this secure form to report this to the Department of Unemployment Assistance (DUA).

You will need to provide your: 

  • Name
  • Date of birth
  • Phone number
  • Address
  • Last 6 digits of your Social Security number
  • Email address
  • Driver's license number or state issued ID number
  • Unemployment Insurance Claimant ID number or PUA Claim ID number (if available)
