How to amend

You may now amend your MAP MCSR on theHealth Professions Licensing Portal. 

Sign in using your username and password. If you need assistance with your username or password, please visit Health Professions Licensing System User Guide for more information.

Once signed in, enter the Board/Program, the License Registration Type, and the MCSR number for the MAP site you wish to amend the information.

Choose either “Start a Service Provider Business Name and Address Change Amendment” or “Start a Service Provider Information Amendment” to update information as applicable.
