The Details of Amend your MCSR
Attention Advanced Practice Providers for Amend your MCSR
Please note we no longer require your supervising physician or qualified healthcare professional name. You will simply attest to having a supervising physician or qualified healthcare professional with written guidelines.
If you qualify for independent prescriptive practice and wish to update your MCSR, or if you are no longer supervised, or your employment status has changed, you can update your attestation online. Please log into your account and submit the amendment application to update your attestation.
How to amend Amend your MCSR
To update the address or drug schedules of your MCSR start by creating an account in the new online licensing system, if you have not done this already. Once you log in you click on Manage My Licenses Applications option, then link your MCSR if you have not done so yet. Once your MCSR is linked, click on show details, the application is there for you to amend your address and drug schedules
Non-terminating changes for Amend your MCSR
Non-terminating actions will retain the MCSR number that the provider currently has in registration. (No fee charged)

Once the Board has updated your legal name change it will automatically update your MCSR.
Physicians and Veterinarians, you must submit the name change application online Health Professions Licensing System (mass.gov) after you change your name on your Board license.
Terminating changes
MCSR’s are non-transferrable. If you are the responsible party that purchases and stores prescription medications at any facility or professional practice, and need to move locations, or leave the location for any other reason, you must terminate the MCSR and apply for a new MCSR for the new address. When you submit the termination form you must provide a written statement, on letterhead, that explains how you disposed of or transferred the prescription stock medications in your possession, including any samples.

These can now be obtained on-line. You can print your own registration by creating an account in the new online licensing system, if you have not done this already. Once you log in you click on Manage My Licenses Applications option, then link your MCSR if you have not done so yet.
Once your MCSR is linked you will see your list of licenses. Choose the license you wish to print, click show details, please select the appropriate document type below then click download and print your wallet card.
