Attention APRNs. for Amend your MCSR

CDTM Pharmacists, Physician Assistant and Advanced Practice Registered Nurses (CNPs, CRNAs and PCNSs): Please note eGov has been updated and we no longer require your supervising physician or qualified healthcare professional name. You will simply attest to having a supervising physician or qualified healthcare professional with written guidelines.

How to amend Amend your MCSR

  1. Fill out the application, making sure to check the field(s) being amended
  2. Mail completed application and all attachments to:

Non-terminating changes

Non-terminating changes

Non-terminating actions will retain the MCSR number that the provider currently has in registration. (No fee charged)

  • Contact information
  • Removing Authorized Drug Schedules
  • Legal name changes-please contact your licensing Board

Once the Board has updated your legal name change it will automatically update your MCSR. To request an updated copy of your MCSR, email us at MCSR@massmail.state.ma.us.

Physicians- Once the Board confirms changes have been made to your Board license, please send the Physician application amendment form to MCSR@massmail.state.ma.us

We will then mail an updated copy of your MCSR.

Terminating changes

Terminating means that a new MCSR number will be issued due to the changes listed. (MCSR application fee charged; new MCSR issued)
