County employees may be required to file a disclosure form to qualify for certain exemptions under the conflict of interest law. This page provides access to disclosure forms, instructions, and information about filing requirements.

Before filing a disclosure, please review the available disclosure forms and instructions to determine which form applies to your situation and where it must be filed, then download and complete the appropriate form before submitting it:

If you have questions about what the conflict of interest law requires, whether you are eligible for an exemption, or how you should complete a disclosure form, please contact the Commission's Attorney of the Day for advice.

Please note: Not all disclosure forms are filed with the State Ethics Commission. Depending on your position, you may be required to file your disclosure with the Commission, your appointing authority, or another designated official. Before submitting a disclosure, please refer to the instructions included with each disclosure form for filing requirements.

How to file

If your disclosure form instructions require you to file with the State Ethics Commission, you may upload your completed disclosure form electronically through the Commission's online filing system.

Don't have an account? Start here:

  1. Click Conflict of Interest law disclosure to open the registration page.
  2. Enter your First Name, Last Name, Email Address, Phone Number, and Organization Name (your public employer).
  3. From the Select Type dropdown menu, select Conflict of Interest Law Disclosure.
  4. Click Submit.
  5. You will receive an e-mail acknowledging your registration request. You will not be able to file electronically until your account has been approved. Once your account is approved, you will receive additional instructions by e-mail for accessing your account and submitting your disclosure.

Already have an account?

  1. Click Conflict of Interest law disclosure to open the login page.
  2. Enter your Username and Password.
  3. Click Log in.
  4. If you have not already done so, download and complete the appropriate form from the County Employee Disclosure Forms list.
  5. Upload your completed disclosure form and complete any required filing information.
  6. Click Submit.
  7. You will receive a confirmation e-mail once your disclosure has been successfully submitted.
