File a state employee disclosure form with the State Ethics Commission
The Details
What you need
Many exemptions to the conflict of interest law require a state employee to file a public disclosure form. To view available disclosure forms and instructions, please refer to the State Employee Disclosure Forms list.
If you have questions about the conflict of interest law, whether you are eligible for an exemption, or how to complete a disclosure form, please contact the Commission's Attorney of the Day for advice.
Please note: Not all disclosure forms listed on this page are filed with the State Ethics Commission. Depending on your position, you may be required to file your disclosure with the Commission, your appointing authority, or another designated official. Before submitting a disclosure, please refer to the instructions included with each disclosure form for filing requirements.
