What you need for How to apply for funeral and burial payment assistance

To apply for funeral and burial payment assistance, you need to

  • Fill out and sign your part of the application
  • Tell the funeral home that you plan to apply, and ask them to fill out and sign their part of the application  
  • Submit all required information, including:
    • Copies of the itemized bills, statements, or invoices of all the services provided and any and all payments/discounts
    • Copies of supporting documentation including a death certificate, receipts of payments, pre-paid funeral and/or burial services contracts, life insurance policy, and bank statements

Most funeral homes in Massachusetts are familiar with the Department of Transitional Assistance's funeral and burial payment assistance application process and can provide the application for you to complete when arranging for services. 

Reminder: you and the funeral home must sign the documents.
