  3. Submit Your Application & Fee Payment

    1. File online via the ePlace Portal.

    2. When prompted, upload your completed and signed supporting forms from Step 2 above as attachments.
       
    3. Pay your fee online with a credit card and submit your application, or you may mail your fee payment to:
