Submit a Request for Official Proposal Form (R109)

Prequalified horizontal construction firms must fill out and submit the R109 Form in order to receive approval to bid on a MassDOT project. Only the most recently revised R109 Form, available in the Downloads section, will be accepted. Outdated forms will be returned.

For MassDOT DCAMM Projects, DCAMM Prime and Subcontractors must attach a copy of their DCAMM Certificate of Eligibility with their R109 Form.

• The deadline for submitting an R109 Form is 1:00 PM the Friday before the day of the bid opening, 1:00 PM Thursday if the Friday is a legal holiday. If submitted late there is no guarantee you will make the bid opening.
• Do not submit an R109 Form for a city, town or other awarding agency (OAA) project.  Contact the city/town/OAA directly for information on how to obtain project plans and specifications.
• If the project value or your total bid amount is above your firm's single class of work limit for the project's class of work, go to Submit a Waiver Request to request a single class limit waiver.
• Go to the DCAMM website for information on applying for a DCAMM Category or for a DCAMM project waiver request.

• Contact MassDOTSpecifications@dot.state.ma.us for questions about plans and specifications.
• Firms should get their Digital ID from Bid Express at least two weeks before the project bid opening date. Contact Bid Express to register for a Digital ID and for questions regarding registration: (888) 352-BIDX (2439), customer.support@bidx.com.

• If your firm is not currently a prequalified horizontal construction firm, go to Submit a Contractor Application for Prequalification.
• If your firm does not have the class of work for the project, go to Submit an Additional Class of Work to request an additional class of work.
