Prescribers
If you are a prescriber, you will need a:
- Professional email address
- DEA Number (must be valid and active)
- Professional License Number
- Controlled Substance ID (MCSR Number)
Note: Enter each credential only once during registration, (i.e. do not enter your Federal DEA number in both the Federal DEA and the Controlled Substance fields).

Pharmacists
If you are a pharmacist, you will need a:
- Professional or primary email address
- Professional License Number
- DEA number, name, and address of your primary work location.

If you are a delegate authorized by a prescriber or pharmacist, you will need a:
- Professional email address
- Name and address of your employer
- Professional email address of the prescriber or pharmacist you are authorized by
