CCM Long-term Services and Supports (LTSS) Assessments

CCM LTSS Assessments

The CCM LTSS Assessments include the Community Long Term Care Needs Assessment and the CCM Individualized Assessment for Skilled Nursing Needs. These assessments are used for all MassHealth members enrolled in or seeking to enroll in the Community Case Management (CCM) program. The CCM LTSS Assessments determine a member’s need for continuous skilled nursing (CSN) services by reviewing and measuring the time it takes to complete all nursing interventions.
