Information for Members Who Need Skilled Nursing or Long-Term Care

We have several resources to help you find a facility for yourself or a loved one. The map of Massachusetts nursing homes can help you find nursing facilities in your area. We recommend that you call the facility directly to ensure that they can meet your needs and to schedule a tour to see the facility in person.

When you find a facility in your area, you should contact them directly. You should ask if they have open beds, make sure they can meet your needs, and schedule a tour to see the facility in person. Resources for Long-Term Care Residents and Families

You can learn about long-term care in Massachusetts, details about individual nursing homes, and resident rights on the Nursing Home Consumer Information webpage. This webpage includes:

  • The Nursing Home Survey Performance Tool, This state resource shows how well each nursing home is rated by the Department of Public Health (DPH). It includes scores for things like management, nursing care, resident rights, food services, and the facility’s environment.
  • Nursing Home Compare, This is a federal resource from the Centers for Medicare and Medicaid (CMS). It gives detailed information about nursing homes across the country, including their CMS five-star rating. CMS gives nursing homes a rating from 1 to 5 stars. More stars mean better quality. The rating looks at health inspections, staffing levels, and quality of care. It’s a helpful tool to compare nursing homes in Massachusetts and find one that meets your needs.
  • Ombudsman Program Contact List, Ombudsman programs help support and speak up for people living in long-term care facilities. They give older adults a way to share complaints and get help with problems so they can live with dignity and respect.
  • Apply for MassHealth Coveragefor individuals in need of MassHealth coverage and information on the application process.  For additional assistance with coverage or eligibility issues, please contact the MassHealth Customer Service center at (800) 841-2900.

EOHHS Long Term Care Discharge Support for Providers

Discharge support is available to assist clinical and care coordination staff from acute care hospitals, psychiatric facilities, and other settings to secure facility-based long-term care placements (e.g., Nursing Facilities, Rest Homes, or Long-Term Acute Care) for patients who are transitioning out of inpatient care.

This service is designed to complement, not replace, the comprehensive discharge planning process required under the Department of Public Health, MassHealth, and the Centers for Medicaid & Medicare Services regulations and contracts. Referring providers are expected to conduct initial outreach to facilities, including submitting referrals and confirming bed availability, prior to contacting the Discharge Support line. This resource is intended exclusively for use by provider staff involved in discharge planning and care coordination.

When to contact Long-Term Care Discharge Support

  • Discharge staff: contact the Discharge Support team when you have exhausted the above resources and are facing challenges trying to find a long-term care placement.
  • For MassHealth members enrolled in a MassHealth managed care plan, contact the member's managed care plan to coordinate discharge planning before you contact the support line.
