Training Objectives
The purpose of this training series is to provide discharge planning staff in acute care and behavioral health hospitals with the information, skills, and resources to support patients experiencing homelessness or housing instability when being discharged.
By the end of this training series, we hope you will:
- Understand the impact of homelessness on an individual’s health and the healthcare system.
- Learn more about the MassHealth requirements regarding discharge planning, specifically with patients experiencing housing instability or homelessness; and
- Enhance your skills in housing problem-solving and navigating and identifying resources for patients experiencing housing instability or homelessness.
The training consists of recorded videos, infographics, and written material. The entirety of the training should take approximately two hours to complete. At any point of the training, you have the ability to leave the page and return to the material. If you do need to leave, please note the section you have last completed.
Upon completion of the training, you will find a request to certify your completion of the training. Please complete this step as documentation to MassHealth that the training was completed
