Attorney forms: For use in all Trial Court departments
Use this form to inform the court that you are appearing in a case to represent a party or yourself. (Updated 12/6/23)
Attorney forms: District Court
District Court form rev. 03/06. Update name or address until the next electronic update of attorney information is received from the BBO. You may also add an out-of-state attorney who has been admitted pro hac vice (for a particular case) by a judge, or a law student who represents a party under S.J.C. Rules.

(Rev. 03/04/2019) This Juvenile Court form is used by attorneys of record to certify that they have provided their clients with information about court-connected dispute resolution services.
The Waiver of Counsel form is used if a party decides to proceed without counsel even though the party has a right to be represented by counsel under Massachusetts law. (Updated 9/8/23)
