Use this Military Affidavit form to inform the court about the military status of a person in your case (50 U.S.C. § 3931). Instructions are included with the form. Revised 4/6/23. If you do not have a lawyer and are representing yourself in a case, simplified instructions for this form are available below.

This form must be completed and filed with the complaint or other initial pleading in all District Court and BMC civil actions seeking money damages.

Use this form to inform the court that you are appearing in a case to represent a party or yourself. (Updated 12/6/23)

This form is to be used in Boston Municipal Court or District Court when proceedings are postponed for diversion under G.L. c. 276A or G.L. c. 111E, § 10, to provide written consent to any terms and conditions imposed, and a written waiver of the right to a speedy trial during the postponement.

Trial Court form

A court must use this form to inform a defendant of his or her obligation to register as a sex offender with the Sex Offender Registry Board prior to a guilty plea to a sex offense or after a conviction of a sex offense. (Updated 12/20/23)

Trial Court form

The Waiver of Counsel form is used if a party decides to proceed without counsel even though the party has a right to be represented by counsel under Massachusetts law. (Updated 9/8/23)

This is a sample only. The form, which must be purchased from the clerk's office of the court where you want to file, is for use in Housing, District, and Boston Municipal Courts.

A summary process case for nonpayment of rent must include this form with the entry package. Before eFiling, complete the fillable fields, then save the document as a flat PDF by clicking on the gray “Save as PDF” button on the top left. Failing to save the document as a flat PDF before eFiling may delay your filing.

Use this form to transfer your case to the Housing Court under G.L. c. 185C, § 20.
