Forms and information relating to bar admission in Massachusetts

All applications for testing accommodations on the bar exam must include Form 1, to be completed by the applicant.

Applicants seeking testing accommodations on the basis of a mental impairment, including ADHD, learning disabilities, or psychiatric disabilities, must submit Form 2, to be completed by the medical professional involved in care/treatment for this condition.

Applicants seeking testing accommodations on the bar exam on the basis of a visual impairment must submit Form 3, to be completed by the medical professional involved in care/treatment for this condition.

Applicants seeking testing accommodations on the bar exam on the basis of a physical disability must submit Form 4, to be completed by the medical professional involved in care/treatment for this condition.

Applicants seeking testing accommodations on the bar exam who have received accommodations in other educational/testing settings must submit Form 5, to be completed by their school/testing agency. Letters confirming the accommodations that have been granted may be submitted in lieu of this form.

Examinees who have previously been granted accommodations on the bar exam in Massachusetts must complete this form in order to renew those accommodations. Eligibility requirements are specified herein.
