Wall Certificate or License verification/Certified Statement request
Applications are now available online through the Eplace Portal.
You will be required to have a portal account. You can create a portal account or log in if you already have one. To create an account select the link Register for an Account and follow the instructions for setting up your account. Then click Manage Licenses, Permits and certificates then File an online application. Select the Board and the type of request and continue.
For license verifications (certified statement, letters of good standing) when specifying the delivery method licensees may select email or mail and in the special instructions indicate whether a form is attached from another state (the form must then be uploaded) to be completed or if the verification is requested via a CLARB record.
There is a fee associated with these requests and may be paid by credit card or electronic check.
