- Federal employer identification number (FEIN) and contact information for your company
- Information for up to twenty (20) of your company’s most recent construction projects of at least $80,000 completed within the past five (5) years per category of work. Also include projects in progress on the combined spreadsheet.

In order to start your online application you must create an account in the DCAMM Contractor Management System portal
This spreadsheet can be used for both completed and in progress project types. Once you have filled in the needed information you will have all the needed fields to type into the projects table in the application.

Once you submit your application, you will receive an email saying your application was submitted to DCAMM. You are able to print an application report at anytime as you will need it for completing the update statement for public bidding.
