The certified verification document provides the following public information on each licensee:
- Name
- License type
- License number
- License issuance date
- License status
- License expiration date
- Disciplinary action information
Please note that the above-listed public information is also available, free of charge, through the Division of Occupational Licensure’s public “Check a License” database.
The Board does not complete licensing verification forms from licensing authorities in other states or jurisdictions, and provides only the above-listed public information to requestors.

This fee may be paid online at the end of the request submission process and is payable by credit card, debit card, or bank account. Please note that a convenience fee may be assessed for all online credit card transactions. There is no fee for online payment by check.

To request or obtain a certified license verification document from the Board, please submit a request through the ePLACE portal. Please note that if you do not already have an account you will be required to create one prior to submitting your request.
When submitting your request, please indicate whether you would like an electronic copy (PDF) e-mailed to you or a hard copy mailed to you or a licensing authority in another state or jurisdiction. If you are requesting a hard copy, please provide a valid mailing address.

In order to maintain an eco-friendly environment, we will no longer accept paper requests by mail. All requests must be submitted online through our ePLACE Portal.
How do I submit my out of state verification form?
Each verification will include a standard letter on our letterhead with the board seal confirming your credentials. It is not necessary to submit any out of state forms.
Can my verification request be expedited?
Due to the high volume of requests received, all requests are processed in the order in which they are received.
What if the requestor is indicating that the verification has not been received?
The system will provide you with a VERIFY record ID. Simply contact the board for a status of your verification, and we may provide a copy of the verification sent to the requestor.
