MassHealth Waiver Programs
MassHealth waiver programs can assist MassHealth members returning to the community.
- The Moving Forward Program within MRC's Home and Community Life Services is for MassHealth members who utilize facility-based care.
- Home and Community Based Services (HCBS) Waivers are MassHealth programs for individuals who would otherwise require care in a facility.
- Frail Elder Waiver (FEW) is a MassHealth program that provides frail elders, aged 60 and older, access to the services and supports they need to live successfully in the community.
Behavioral Health Community Partners Supports & DMH Case Transition Management Team for Nursing Facility Residents
- The Behavioral Health Community Partners (BH CP) Program and the Department of Mental Health (DMH) Transition Case Management Team work in a coordinated manner to provide additional support to specific population within nursing facilities. Community Transition Liaison Program
- The Community Transitions Liaison Program (CTLP) provides focused support for all NF residents who are 22 and older, regardless of insurance type, who are interested in transitioning to the community. More information about this program can be found in Nursing Facility Bulletin 179.
