If you are a MassHealth Standard or MassHealth CommonHealth member and you need in-home nursing services for more than two continuous hours a day, you may qualify for CSN services through the Community Case Management (CCM) program managed by the University of Massachusetts Medical School. CCM is responsible for assessing and approving CSN services and other long-term services and supports. Contact CCM by phone at (800) 863-6068 or e-mail at commcase@umassmed.edu.
