Dental Licenses

Learn how to apply for initial licensure as a dentist and/or renew an existing dental license. This page doesn't apply to initial licensure as a limited license dental intern, limited license dental faculty member, dental hygienist, or dental assistant.

Here you can apply for your initial license as a dentist by examination or by credential. You can also apply for your individual anesthesia/sedation permit and/or a facility anesthesia/sedation permit. These resources are specific to dentists, limited license dental interns, and limited license dental faculty members. These resources don't apply to dental hygienists or dental assistants.
