Family Caregiver Support Program Eligibility

The Family Caregiver Support Program is free. This program can help you care for a member of your family. A Caregiver Specialist will create a plan and give you tips and support.

Eligibility Guidelines

You are eligible for the program if you are a qualifying caregiver caring for the following individuals:

Caregiver Individual Being Cared For
  • Adult family member or other non-paid caregiver AND
  • Age 18 or older
Individual age 60 or older, OR individual of any age who is living with Alzheimer's disease or a related dementia
  • Grandparent or other relative (not parents) AND
  • Age 55 or older
Child under the age of 18
  • Grandparent or other relative (including parents) AND
  • Age 55 or older
Adult with a disability who is 18 through 59 years old
