Senior guide and application for health care coverage

The Senior Guide to Health Care Coverage makes it easier for you to understand health coverage available in Massachusetts from MassHealth or the Health Connector. This booklet answers important questions that you may have about how to get health care benefits and the programs that are available.

This guide is for Massachusetts residents who are

• 65 years of age or older and living at home; or any age and are in or are waiting to go into a long-term-care facility, or
• Eligible under certain programs to get long-term care services to live at home, or
• 65 years of age or older, disabled, and are either working 40 or more hours a month, or are currently working and have worked at least 240 hours in the 6-month period immediately before the month of the application, or
• Not working, or
• Applying for Health Connector plans

Application for Seniors and People Needing Long-Term-Care Services

This is your application for health coverage if you live in Massachusetts, and you

• are 65 years of age or older;
• are not otherwise eligible for MassHealth;
• are not getting Medicare; and
• do not have access to an affordable health plan that meets the minimum value requirement

Renewal Application for Certain Seniors Living in the Community

The Renewal Application for Certain Seniors Living in the Community is intended for certain MassHealth members and their spouses, if they are married, who are 65 or older and living in the community.

Renewal Application for Seniors and People Needing Long-Term-Care Services

This is your application for health coverage if you live in Massachusetts, and you

• are 65 years of age or older;
• are not otherwise eligible for MassHealth;
• are not getting Medicare; and
• do not have access to an affordable health plan that meets the minimum value requirement
