Please note that there isn't a form for a Separation Agreement. A Separation Agreement is a written contract that you write with your spouse. It explains how you will handle any issues related to your divorce. These issues may include:  

  • Child support
  • Parenting time
  • Spousal support
  • Child custody
  • Dividing shared property

Joint petition for divorce pursuant to G.L. c.208, § 1A (CJD 101A)

Complaint for Support Pursuant to G.L. c.209

Complaint for support pursuant to G.L. c. 209 § 32F (CJD 107) 

Form to use to request support and/or health insurance benefits for children, if you are living apart from your spouse. Form number CJD-107, revised August 2009.

Request for copies form (PFC 18) 

Use to order a copy of court record, such as a divorce judgment or a decree of guardianship. Request form & payment must be mailed to the court division where the case is/was docketed.
