What you need

Who can use this application

Use this application if you live in Massachusetts and:

  • You are aged 65 or older and living at home, and:
    • Not the parent of a child under 19 years of age who lives with you, or
    • Not an adult relative living with and taking care of a child younger than 19 years of age when neither parent is living in the home, or
  • You are an individual of any age and need long-term care services in a medical institution or nursing facility
  • You are an individual who is eligible for certain programs to get long-term care services to live at home (for more information on long-term care services, please see the Senior Guide to Health Care Coverage)
  • You are a member of a married couple living with your spouse, and
    • Both you and your spouse are applying for health coverage
    • There are no children under 19 years of age living with you
    • One spouse is 65 or older and the other spouse is under 65

What you need

To apply, you may need to provide the following information and documents.

  • Social Security numbers, if you have them, for every household member who is applying
  • Proof of income and assets
  • Proof of any health insurance that you are currently enrolled in or have access to
  • Information about or proof of citizenship/national status or immigration status

How to apply

To complete the Senior application on your own:

  1. Print the Application for Health Coverage for Seniors and people Needing Long-Term Care Services and any needed supplements
  2. Complete, sign, and date application, and any needed supplements
  3. Collect the required documents (such as income, assets, citizenship, or immigration, etc.)

  1. Print the Application for Health Coverage for Seniors and people Needing Long-Term Care Services and any needed supplements
  2. Complete, sign, and date application, and any needed supplements
  3. Collect the required documents (such as income, assets, citizenship, or immigration, etc.)
  4. Fax the filled out application, any needed supplements, and required documents to (617) 887-8799

To apply in person, you can go to any of the 4 enrollment centers listed below. Enrollment centers are open Monday through Friday from 8:45 a.m. to 5 p.m. Do not mail or fax an application to any of these enrollment centers.

Make sure to bring with you the required documents (such as income, assets, citizenship, or immigration, etc.).

Next steps

  1. When we receive your application

    • We review the application
    • Write to you or call you if we need more information
    • Make a decision about your eligibility and send you a written notice about this decision. 

     

  2. If you have paid for medical services during the eligibility period

    • Show the written MassHealth notice to your health care provider right away
    • If the health care provider determines that MassHealth will pay for the services you already paid for, the provider could refund what you paid
