PFML workplace poster, notices, and rate sheets for Massachusetts employers

All Massachusetts employers (including employers with private or self-insured benefits plans) are required to distribute certain information to their employees under the Paid Family and Medical Leave (PFML) law.

The notices and rate sheets are provided in Word format for your convenience. DFML disclaims any responsibility for modifications made to these forms and cannot guarantee that a form that has been modified from this original version will be compliant.

Workplace Poster

All Massachusetts employers must display a workplace poster prepared or approved by the Department of Family and Medical Leave (DFML) that explains the benefits available to your workforce under PFML law. You must exhibit this poster at your workplace in a location where it can be easily read. 

The poster must be available in English and each language which is the primary language of 5 or more individuals in your workforce. DFML provides translations in English and 12 other languages.  The employer is responsible for providing translation with regard to any language not provided by DFML.

Notice for workforce with 25 or more covered individuals

This notice is for employers who have 25 or more Massachusetts employees and covered self-employed individuals. Use it to inform the employees and covered self-employed individuals in your workforce about their rights and obligations under the PFML law.

If you employ covered self-employed individuals, you may use either these notices below or the notices in the sections above to notify those workers. 

This form is for employers who have 25 or more self-employed individuals. Use it to inform any self-employed individuals you engage about their rights and obligations under the PFML law.

The notice must be available in English and each language which is the primary language of 5 or more individuals in your workforce. DFML provides translations in English and 12 other languages. The employer is responsible for providing translation with regard to any language not provided by DFML.
