Rule 28A
The last page of every paper served in accordance with Mass.R.Civ.P. 5(a) or Mass.R.Dom.Rel.P. 5(a) shall contain a brief statement showing the date on which and manner in which service of the paper was made on each other party, with the name and address of each person served. The statement may be in the following form:
I hereby certify under the penalties of perjury that a true copy of the above document was served upon the following people at the following addresses by mail (by hand) on (date):
(Names and addresses),
(Signature).
Reporter's notes
(2012) Although most attorneys include a certificate of service with each motion or other paper served, neither the Mass.R.Civ .P. nor the Mass.R.Dom.Rel.P. require one unless service is challenged. This proposed rule is similar to Superior Court Rule 9B and so should already be familiar to most attorneys.
