For use in Land Court (revised 12/2023). This form is used by parties and their attorneys when participating in mediation through the Land Court’s in-house mediation program.

For use in Land Court (revised 5/31/23). Complete this form to submit a request to the court to appoint a specific appointee in a pending Land Court case. All fee generating appointments must comply with S.J.C. Rule 1:07.

Courts are starting to use a system called FTR, but it is not yet available in all courts. If FTR is available for your case, audio recordings are accessible online for $10.00 at https://us.court.fm. Otherwise, use this form to order a CD.

For use in Land Court (rev. 1/22/26).

For Land Court (rev. 3/1/26). Must be based on claim(s) listed in G.L. c. 185, § 3A (a)-(d) and project must involve 25 or more dwelling units and/or construction or alteration of: 25,000 sq. ft. or more of gross floor area; Class I renewable energy generating source (defined); and/or energy storage system (defined).

For use in Land Court (effective 10/1/2021, last rev. 9/22/2021). Complaint to declare provisions void that restrict the transfer or use of property based on race, color, religion, national origin or sex, pursuant to G.L. c. 184, § 23B and Land Court Standing Order 2-21, effective October 1, 2021. No filing fee due.

For use in Land Court (revised 5/31/23). Fee Generating Appointees in a Land Court case may submit this form to comply with the statement of payment requirement under § 7 of S.J.C. Rule 1:07.

For use in all courts. All filers must mark on this form all applicable confidentiality provisions, and identify the precise location of the confidential information within the document being filed. Submitting this form does not, by itself, guarantee compliance with any additional rules for handling impounded info.

Use this Military Affidavit form to inform the court about the military status of a person in your case (50 U.S.C. § 3931). Instructions are included with the form. Revised 4/6/23. If you do not have a lawyer and are representing yourself in a case, simplified instructions for this form are available below.

Use this form to inform the court that you are appearing in a case to represent a party or yourself. If you are appearing in a case to represent yourself and would like to receive court notifications by email, you must include your email address on this form. (Updated 12/6/23)

For use in Land Court (09/2025). Any party can use this form to request that the Court take action in a case. Select the action requested or describe it in the space provided. Provide supporting information. Statements are made under penalty of perjury.

For use in Land Court (09/2025). A Plaintiff can use this form to request that a Defendant be defaulted under Rule 55(a) of the Massachusetts Rules of Civil Procedure for failure to respond to a Complaint. Identify the Defendant and provide supporting information. Statements are made under penalty of perjury.

For use in Land Court (09/2025). A Plaintiff can use this form to dismiss a case under Rule 41(a)(1)(i) of the Massachusetts Rules of Civil Procedure. This can only be filed before the adverse party serves an answer or a motion for summary judgment, whichever comes first.

The Land Court does not accept for filing any Servicemembers Civil Relief Act cases not accompanied by the Mortgagee's Affidavit under MGL c.244, § 35A, as amended by St. 2010, c.258, § 7. TL 1-5 (8-2025). For use in the Land Court for filing a new Tax Lien Complaint to foreclose property owners’ rights of redemption pursuant to G.L. c. 60, § 65.

As of Nov. 1, 2024, parties must use the notice of filing prepared by the Department of Revenue, per G.L. c. 60, § 75, as amended by c. 140 of the Acts of 2024, § 96. Plaintiff must record or register the notice in the local Registry of Deeds or Land Registration District, and then file a copy with the Land Court.

For use in Land Court (10/2016).

LC-TL-MW (11/24). This form is mandatory for use in Land Court Tax Lien (TL) cases when a plaintiff moves to withdraw the Complaint to Foreclose Rights of Redemption previously filed in the case.

LC-TL-MOHRG (02/24). This form is mandatory for use when filing any motion and request for the court to schedule a hearing in Land Court Tax Lien (TL) cases. If needed, attach supporting affidavits or documentation to the motion form.

LCTL-ANS-CS (10-2021). For use in Land Court.

The “Guide and File” program can help a defendant complete and file a Tax Lien Answer form online. Guide and File guides the user through questions to complete the form. The completed Answer form can then be filed at the Land Court either electronically (eFile) or in person/by mail (on paper).

Mandatory form to be completed by plaintiff in all tax lien cases to move the case beyond the first-tier citation phase.
