Helping People Return to the Community

The Money Follows the Person (MFP) Demonstration helps elders and people with disabilities move from facility-based care back to the community. MFP provides supports and connects members to home- and community-based services (HCBS) designed to help individuals transition from nursing facilities and other long-stay hospital settings to the community.

Although specific supports that were available through the MFP Demonstration to help members transition have ended, the grant continues to fund community based services. In addition, a program formerly related to the grant, the HUD 811 Project Rental Assistance Program, will continue to identify housing opportunities for people leaving nursing homes and other long-stay facilities.
