This series of documents serves to provide support to long-term care facilities (LTCFs) in providing care for residents diagnosed with an opioid use disorder (OUD) or stimulant use disorder (StUD) who are on medication for opioid use disorder (MOUD).

Introduction

This section introduces the background of Opioid use disorder (OUD) and trends in America. Medication for opioid use disorder (MOUD) is introduced and the rest of the sections of the toolkit are outlined.

Tip 1: Understanding Opioid Use Disorder

This section provides guidance to LTCFs to help them create a supportive care environment by understanding OUD and how dispelling stigma and myths can foster better care for residents.

TIP 2: Creating a Therapeutic Environment

This section identifies several interventions LTCFs can implement to help foster a therapeutic environment that meets residents’ linguistic and cultural needs.

Tip 3: Organizational and Workforce Approaches to Person-Centered Care

This section helps LTCFs establish a multi-layered, interdisciplinary, person-centered approach to supporting residents on MOUD that meets residents’ linguistic and cultural needs.

Tip 4: Demonstrated Competencies

This section helps LTCFs identify key competencies that staff should demonstrate and understand in order to better care for residents on MOUD.

Tip 5: Community-Wide Resources and Partnerships

This section helps LTCFs become familiar with and learn how to connect with treatment partners and community resources in order to better care for residents on MOUD.

Tip 6: Transitions of Care

This section helps LTCFs establish thoughtful and safe transitions from the hospital to the LTCF while maintaining communication with the resident’s Opioid Treatment Program(s) or Office-Based Opioid Treatment Program(s) as well as safe transitions from the LTCF after discharge.

Appendices

The 13 appendices supplement the information in the toolkit and serve to illustrate the concepts throughout.
