Acquired Brain Injury (ABI) and Moving Forward Plan (MFP) Waivers

Helping MassHealth members move from a nursing facility or long-stay hospital back to their community.

Home- and Community-Based Services Waivers are for MassHealth-eligible people who would otherwise need facility-based care. The ABI and MFP waivers are for Massachusetts residents living in nursing facilities or long-stay hospitals, who may be able to move out and live more independently with waiver services and supports. Every waiver participant has a waiver case manager or service coordinator to coordinate their services to support successful community living.
